A PCB or Printed Card Board by itself has nothing to do with chips. There were PCBs already before chips existed. A PCB is a fiberglass plate with copper traces to connect all the components together.

Should there be a reference GND under the feed trace? Is it good to have a completely gnd plane for the layers below the reference plane?

It is a cheap technique to add a chip to a mass production device, because it does not require all the soldering and casing needed for "regular" chips.
