Initial Form LM-1 Every labor organization subject to the LMRDA, CSRA, or FSA is required to adopt a constitution and bylaws and attach a copy with an initial Form LM-1 filed with OLMS. The initial Form LM-1 must report certain information concerning the structure, practices, and procedures of the labor organization.
