Ico is the latest victim of a mysterious curse, manifested by a pair of horns that grew from either side of his head. Subsequently, he has been blamed for any misfortune that ever befell his home village, dooming him to a life of alienation. The game opens as Ico is being led away, hands bound, to a decaying castle overlooking the sea.

The player controls Ico, a boy born with horns, which his village considers a bad omen. After warriors lock him in an abandoned castle, he frees Yorda, the daughter of the castle's Queen, who plans to use Yorda to extend her life.

Some of the men have horns on their helmet. The men proceed down a winding path towards sea level, get in a small boat, and row silently towards the castle. They have a prisoner with them: Ico, a boy with an immediately identifiable characteristic: two large horns protruding from his head.

Ico was a boy born with horns, which is considered a curse in his land. He was brought to the castle by three masked men and forced into a coffin to presumably die. After they leave, however, a tremor caused by the castle's many aged moving parts dislodges Ico's coffin from the wall, breaking it open and starting his quest.

Ico is also the name of the playable character, a young boy with horns. His aim is to escape a dark crumbling castle in which he was imprisoned. Early on he rescues a young girl called Yorda from a cage suspended from the castle ceiling and they both must escape while avoiding being captured by strange dark creatures which also inhabit the castle.
