Bilateral symmetrical uptake in the sacral ala (the Honda sign), represent sacral insufficiency fractures. There is also uptake in the left pubic bone where there is another fracture which was occult on x-ray.

The H-shaped pattern he described has become known as the Honda sign. The horizontal and vertical lines of increased uptake seen on the bone scan (Figure 1) indicate the areas of insufficiency fracture and resemble the letter H of the Honda automobile emblem (Figure 2).

Honda sign, which is classically described in bone scans in cases of SIF, was found in FDG PET-CT in our case. This suggests SIF should be suspected when similar FDG uptake pattern is encountered and may help in early detection and management of SIF.
