2023年1月25日 · "A-fib with RVR" is actually clearly defined, it's A-fib with a ventricular rate greater than 100 bpm. We tend to specify the RVR as a reference to hemodynamic compromise, but that's not correct, there is A-fib with RVR and there is A-fib with RVR resulting in hemodynamic compromise.

AFib due to loss of pre-systolic atrial kick loses out on some amount of diastolic filling, so a patient with AFib will be more dependent on passive diastolic flow, which is an inverse function of heart rate (since faster heart rate means less diastolic flow).

2022年5月21日 · 2 reasons. 1) many of our patients have COPD or asthma, so beta blockers have some risk. 2) Most importantly, our experience is that dilt works better. It is faster and more effective in afib w/ rvr that we see. Others have already posted the literature.
