The sulcus test is used to assess the glenohumeral joint for inferior instability, due to laxity of the superior glenohumeral ligament and coracohumeral ligament. It was first described by Neer and Foster in 1980 to examine multidirectional instability.

The Sulcus Sign is an orthopedic test to assess inferior shoulder instability or laxity of the inferior capsule of the glenohumeral joint. Eshoi et al. (2018) have evaluated the inter-rater reliability of this test and the authors have found a moderate degree of reliability with a Kappa k of 0.48.

The sulcus sign is an orthopedic evaluation test for glenohumeral instability of the shoulder. With the arm straight and relaxed to the side of the patient, the elbow is grasped and traction is applied in an inferior direction.

Sulcus Sign. The sulcus sign is used to evaluate inferior glenohumeral joint instability. The patient is seated or standing with the arm relaxed in shoulder adduction. The patient’s forearm is grasped by the examiner and a distal traction force is placed through the patient’s arm.

Sulcus test is considered positive when a sulcus sign is seen when the examiner applies a downward force applied at the elbow while the arm in neutral rotation and resting at the patient’s side. Shoulder instability occurs when the head of the upper arm bone is forced out of the shoulder socket.

Sulcus Sign (Inferior Glenohumeral Instability) Purpose : To determine if there is a presence of inferior glenohumeral instability and/or laxity (1). Patient position: Standing or Sitting upright with back unsupported and with affected arm relaxed by the side.
