In DnD this is seen as a massive failure as opposed to a nat 20 which is a massive success (both are subject to the whims of the Dungeon master, some dont' do this). Like say you want to jump from a roof to another roof to sneak in a bedroom. THe DM makes you roll an athletics and stealth check.

Before anything else happens, a nat 1 should be confirmed with a 2nd nat 1. 1 in 400 odds are a bit more rational for complications to happen on top of, as long as they aren't ongoing beyond a long rest. Anything ongoing needs to be doubly confirmed.

Once I had a monk accidentally hit himself in the jaw and stun himself for two rounds. Once they were fighting in a giant nest, and every time anyone rolled a one, one of the eggs got broken. Nat 1 penalties can be really situational.
