2016年10月19日 · Upon using Mangekyou Sharingan, the user's eyesight will gradually gets worse. The more the frequency and the more strain the user puts on the eyes, the faster the process will be. Using Mangekyou Sharingan techniques such as Amaterasu, Tsukuyomi, and Susano'o puts great strain on the eyes, much much greater than just activating it.

(Granted not all of them have used all the abilities.) However, Shisui and Obito's sharingan have unique Mangekyou abilities (Komoamatsukami and Kamui respectively).

In-universe explanation: As you said, Kakashi was either unaware of or unable to control his Mangekyou Sharingan. I tend to think the former is the case, since he passed out as soon as he awoke his Mangekyou, and probably had no memory of awakening it (Kakashi's page on Naruto wiki seems to agree with this explanation). But I don't understand what the special ability Madara got from his Mangekyou Sharingan. I don't think Susanoo and controlling Nine Tails are special abilities because they are basic abilities of MS.

My guess is it places too much stress on the Optic Nerve, with the Eternal Mangekyou simply not using the Optic Nerve at all but rather a self genjutsu to transmit directly to the brain.
