This patient experienced blunt eye injury after bungee cord recoil. Color fundus montage photograph of the right eye shows a macular hole and peripapillary choroidal ruptures extending into the macula. There is subretinal hemorrhage and fluid inferior to the macular hole and in the peripapillary region.

There was also atrophy superiorly. In the infratemporal macular there was a round fibrotic scar with signs of previous intraretinal fluid. OCT OS showed a pigment epithelial detachment with associated subretinal and intraretinal fluid. The patient was diagnosed with a choroidal neovascular membrane and treated with intravitreal avastin.

Notable findings include a full-thickness macular hole and multiple arc-shaped choroidal ruptures in the inferotemporal macula. There is also a large area of hypopigmentation secondary to commotio retinae, or loss of retinal transparency secondary to photoreceptor disorganization.
