OX-2 membrane glycoprotein, also named CD200 (Cluster of Differentiation 200) [5] is a human protein encoded by the CD200 gene. [6] In humans, the CD200 gene is located on chromosome 3 in proximity to genes encoding the other B7 proteins CD80 / CD86 .

We show that CD200 is uniformly expressed in chronic lymphocytic leukemia (CLL) and absent in mantle cell lymphoma (MCL). Importantly, expression of CD200 is retained even in those CLLs with immunophenotypic aberrancies, making CD200 a particularly useful marker for discrimination between these cases and MCL.

Through this CD200/CD200R signaling pathway, CD200 exerts its immunosuppressive effects by inhibiting natural killer (NK) cell activation, cytotoxic T cell functions, and M1-polarized macrophage activity, while also facilitating expansion of myeloid-derived suppressor cells (MDSCs) and Tregs.

In this review, we focus on the role of CD200 expression in the differential diagnosis of mature B-cell neoplasms and on the prognostic significance of CD200 expression in CLL, where conflicting results have been published so far.

CD200 is a transmembrane protein related to the B7 family of costimulatory receptors involved in T-cell signaling and likely plays a role in physiologic immune tolerance. It is normally expressed on lymphoid and neuronal tissue, and its receptor, CD200R, is expressed on antigen-presenting cells and T-cells.
