As described in the 2023 Advance Notice and Rate Announcement, CMS has revised the CMS- Hierarchical Condition Category (HCC) End-Stage Renal Disease (ESRD) model used to pay for beneficiaries enrolled in Medicare Advantage plans and certain demonstrations (including the Medicare- Medicaid Plans (MMPs)).
First, a CMS Hierarchical Condition Categories (CMS-HCC) model generates a risk score for each beneficiary that summarizes each beneficiary’s expected cost of care relative to other beneficiaries.
