Phenytoin and fosphenytoin have long been recommended as second-line treatments of SE [6]. They are associated with numerous side effects including local reactions of the administered vein, allergic dermal reaction/hepatitis, arrhythmia, and hypotension [7].

Our data suggest that FOS may produce more cardiac toxicity than previously thought. Clinicians should consider administering intravenous FOS in a monitored setting for selected high-risk patients.
